- Description
Test Description and Utilization:
- Useful for diagnosing immune hemolytic anemia not detectable by routine methods.
- Testing will be performed by American Red Cross Pomona Immunohematology Reference Lab.
- This workup includes Monospecific anti-IgG and anti-C3 by standard methods, DAT anti-IgA, DAT anti-IgM, 4C DAT anti-IgG (low affinity IgG), and Polybrene DAT anti-IgG (low concentration IgG).
Ordering Requirements:
- Laboratory Medicine Resident (LMR) approval is required. Contact the LMR via the paging operator at (206) 598-6190.
- Patients should have a concurrent order for a DAT or have a documented negative DAT in the UW Medicine system within the last week.
- If the recent DAT is negative, then the LMR will approve sendout testing.
- If the recent DAT is positive but the provider would still like to send this testing for additional information, the clinical utility can be determined by the resident.
- Providers must submit a completed American Red Cross Immunohematology Consultation Request Form with all orders.

Standard tube technique with anti-IgG, -IgM, and -IgA reagents, Polybrene (resuspension and anti-IgG), and/or cold LISS wash technique as needed.

- Interferences and Limitations
Cautions: The presence of recently transfused RBCs may compromise results for the purpose of diagnosing autoimmune hemolytic anemia.

Processing
- Receiving Instructions
Store and transport whole blood at ambient temperature if shipping same-day (preferred).
- If sample is drawn concurrently with in-house DAT, the specimen should refrigerated until it can be sent to ARC, pending the in-house DAT results and LMR approval.

Sendouts:
- Providers must submit a completed ARC Immunohematology Consultation Request Form.
- Affix CAST Billing/Results labels to the form in the "Submitting Facility" section.
- Contact the American Red Cross Southern California Lab at 909.859.7410 prior to sending a sample. Record the name of the person contacted and date/time of notification on the ARC IRL request form.
- Ensure that sample labels include patient name, patient ID number, and date collected.
- If shipping the same day of collection, send samples at ambient temperature via FedEx Priority Overnight. If shipping the day after collection, send refrigerated.
- If shipping on a Friday, select "Saturday Delivery" when creating the FedEx label.
