Reference Values: < or =300 mcg/L Interpretation:
Increased excretion is consistent with renal tubular damage.
Beta-2 microglobulin excretion is increased 100 to 1000 times the upper limit of the reference interval in cadmium-exposed workers.
- Interferences and Limitations
Cautions:
Degradation of beta-2 microglobulin occurs at pH less than 6. At the time of urine collection, 1M sodium hydroxide needs to be added as preservative to achieve a pH between 6 and 8.
For diagnostic purposes, the results obtained from this assay should always be used in combination with the clinical examination, patient medical history, and other findings.

Collection Instructions:
- Patient should empty bladder.
- Have patient drink at least 0.5 liters of water.
- Within 1 hour, collect a random urine specimen.
- Forward to the laboratory immediately.
- Handling Instructions
Outside Laboratories:
- Add 1M NaOH as preservative to random urine collection within 2 hours of collection. This preservative is intended to achieve a pH of between approximately 6 and 8.
- Freeze urine specimen at -20°C while awaiting shipment. Transport on dry ice.
Stability: Frozen (preferred): 14 days; Refrigerated: 48 hours; Ambient: Unacceptable.
Reject Due To: Specimen with pH <6.

- Receiving Instructions
SPS: Decant in Sunquest to create an aliquot CID label, transfer urine to a screw-cap plastic vial, and forward the aliquot to the Chemistry manual bench as soon as possible. The pH of the specimen must be adjusted within 2 hours of collection.
Chemistry:
- Add 1 M NaOH to achieve specimen pH of between 6 and 8.
- Freeze urine at -20°C (place in Sendouts Mayo Frozen Rack).
