Description

This order is specifically for sending IgM testing on the "Measles IgM Draw and Hold" (Measles IgM Draw and Hold Sample [MEVDRH]) sample collected as part of the ACUTE MEASLES ORDER PANEL. The sample for this order should already be collected and stored in Virology at the Central Lab. The sample is stored for 2 weeks from the date of collection to facilitate this add on order.

Useful For: This test is used for suspected cases of acute disease when PCR testing is negative and clinical suspicion is high. This test allows for Measles IgM testing to be added on to the serum sample collected as part of the ACUTE MASLES ORDER PANEL.

Requirements for Testing:

Coordination with Infection Prevention is strongly recommended for deciding to send Measles IgM testing. At FHCC, orders should only be placed by Infection Prevention.

  1. Obtain approval from the Local Health Jurisdiction (LHJ) epidemiologist prior to placing this order.
    • Ordering providers must consult the LHJ epidemiologist of the patient's county of residence to obtain approval. If testing is recommended, LHJ staff will notify the Washington State Department of Health (DOH) Office of Communicable Disease Epidemiology (CDE).
    • See "Resources" below for contact information.
  2. Submit a LADDON (Add on) order in Epic for "Measles IgM (MEVIGM)." In the order comments, include the name and phone number of the LHJ epidemiologist as documentation of approval.
    • For example: "Approved by LHJ Dr. X, 206-XXX-XXXX."
    • For Infection Prevention ONLY: Enter "Approved by IP; LHJ approval in progress" in Comments for Lab field.

***Failure to provide documentation of approval will result in order rejection.***

UWPC and Ambulatory Clinics: Providers must obtain approval from the LHJ as described above.


Reference Lab Clients: This add-on test is not available to reference lab clients. Additionally, due to the approval requirements and limited sample stability, clients are advised to send samples directly to the WA Public Health Lab. ***Notification 10/10/2024: Due to a nation-wide reagent shortage, Measles IgM testing is not currently performed at WA PHL. Approved orders may still be sent to PHL, who will forward specimens to CDC for testing with a turnaround time of 7 days.***

Note: Optimal timing for Measles IgM testing depends on exposure, symptom, and/or immunization history. Consultation with the Local Health Jurisdiction is required before ordering Measles IgM testing.

Transport:

  • Suspected measles specimens must be enclosed within two biohazard bags (double-bagged) to ensure proper containment, especially if sent through the pneumatic tube system.
  • Samples for this add-on test have already been processed and stored in the virology lab, but double-bagging remains recommended.
Handling Instructions

The sample for this test is collected as part of the Acute Measles Order Panel in Epic available to the UW Medicine system.

The sample is stored frozen (-20°C) in Virology at the Central Lab for 2 weeks following collection.

This specific test can only be ordered as an add-on to the Acute Measles Order Panel and cannot be ordered by outside laboratories. Outside laboratories should see Measles Antibody IgM to Washington State Lab (Sendout) [913] for Measles IgM ordering and collection recommendations.

The serum sample for this test will be stored in Virology in the Central Lab at -20°C.

***SPS: Review the Epic requisition (or in Sunquest modifier) for notice of approval. Approval by LHJ epidemiologist is required.

Following approval, coordinate with the Virology Lab to retrieve the sample for testing.***

    • Manual requisitions are no longer accepted by WAPHL.
  • Order Test: Measles IgM, EIA under "WA Virology." Enter a separate order for each sample type received.
    • Not all specimen types may be indicated. Place orders and send samples as received. Do not delay orders in anticipation of receiving additional specimen types.
  • Print the resulting requisition and package it with the sample.
  • Send samples to WPHL via DLMP or Delivery Express courier.

Stability: This sample should be frozen at -20°C. Refrigerated samples are only accepted within 48 hrs of collection. Ambient: Unacceptable.
