Description

Clinical Utility: Analysis of acylcarnitines is useful in diagnosis of defects in fatty acid beta-oxidation and various disorders of organic acid metabolism. Acylcarnitine analysis is often used in conjunction with urine organic acid analysis in the diagnosis of these disorders and in followup of presumptive positive newborn screening results.

    Test Description: Measurement of acylcarnitines by ESI-MS/MS using stable isotope internal standards. Free carnitine is also reported. To order Free/Total Carnitine only, see separate test Carnitine Group (Sendout) [RCARNG].

      Ordering Requirements: Include relevant clinical information (drug therapy, family history, etc.) in the Epic order comment to ensure proper interpretation.

        Ordering Note: Plasma is the preferred sample type, especially for follow-up testing of an abnormal newborn screen. See Acylcarnitine Profile, Plasma/Serum (Sendout) [RACYLP]. Dried blood spot testing should only be performed on neonates when blood volume is a concern.

        Materials: Newborn Screening Collection Cards are available through Materials Management. Specify Stock Item: #61118. The laboratory does not stock collection cards.

          Collection:

          • Cleanse the site with a sterile alcohol pad and allow alcohol to dry completely.
          • Perform capillary puncture using a sterile lancet. Wipe the first drop of blood with a sterile guaze pad.
          • Fold back the protective flap on the card to expose filter paper. Do not touch or handle the filter paper before or after applying blood.
          • Lightly touch filter paper to the large blood drop, allowing blood to soak through the filter paper and saturate the circle completely with a single application. Apply blood to one side of the paper only.
          • Very gentle, intermittent pressure may be applied to the area surrounding the puncture site. Do not squeeze the puncture site.
          • Continue with collection until at least 3 of the 5 circles are completely filled with blood.
          • Allow collection card to dry completely on a clean, dry, non-absorbent surface, then replace the protective flap and store in the provided envelope.
            • Do not store samples in plastic.
